We have a co-management model where we co-manage patients with primary and specialty care. The physicians see patients for 5 hours per day and help our community based primary care providers manage their patient population. We schedule 90 minutes for a new patient consult and our follow up appointments are 30 minutes long. Our providers share patients and continuously consult and provide updates on the patient’s progress or any concerns with one another. We are part of the only Children’s Hospital in the State. We have access to all pediatric specialties for consult on our patients with medical needs, such as cardiology, GI, and neurology. Because of our EMR, EPIC, we are able to see the patient’s medical history and have a clear picture of their needs. https://youtu.be/N1TGE35HW3g
